401 KAR 52:050.Permit application, registration application, and compliance forms.
Section 1.
Applicability.(1)
An applicant shall use forms DEP7007 AI through HH, to apply for a permit, permit revision, permit renewal, or registration pursuant to 401 KAR 52:020, Section 4(1); 52:030, Section 4(1); 52:040, Section 4(1); or 52:070, Section 7(1), as applicable.(2)
An applicant may use previous versions of the forms incorporated by reference in this administrative regulation until July 1, 2019.Section 2.
